Scientific Background: The Birth of Orthodontics

1899 — The Angle classification. Edward H. Angle defined malocclusions as Class I, II and III according to the position of the upper and lower molars relative to each other. The classification turned orthodontics from a descriptive craft into a measurable discipline and remains the shared language of diagnosis. Angle founded the first orthodontic school in 1900.

1945 — The tooth positioner idea. Harold Kesling described a flexible appliance that moved teeth in small steps: using a slightly different tray at each stage could carry the teeth gradually to their target. The idea was sound, but producing every stage by hand was impractical.

1997-1999 — Clear aligners in series production. Three-dimensional scanning and computer-aided manufacturing made Kesling's idea feasible; the first clear aligner system was cleared in 1998 and came into use in 1999.

How do teeth move?

A tooth is not attached directly to bone; between them lies connective tissue called the periodontal ligament. Under continuous light force, bone is resorbed on the pressure side and formed on the tension side, and the tooth slowly changes position. What matters is the continuity of the force rather than its magnitude — which is why daily wear time is critical in aligner treatment.

The retention phase after treatment follows from the same biology: until the new bone matures, teeth tend to drift back towards their former positions.
