Scientific Background: Research in Paediatric Dentistry

1945 — Fluoridated water. Grand Rapids, Michigan became the first city to add controlled fluoride to its drinking water. In the years that followed, a marked fall in the prevalence of decay among children was recorded — the first large-scale evidence for a preventive approach in children's dental health.

1967 — Fissure sealants. Cueto and Buonocore showed that applying a flowable resin to the deep grooves on the chewing surfaces of molars prevented decay. Those surfaces are too narrow for a brush bristle to reach, so they need sealing rather than cleaning.

Why treat baby teeth at all?

The "they will fall out anyway" view is problematic for two reasons. First, primary teeth hold space for the permanent ones; when a primary molar is lost early, neighbouring teeth drift into the gap and the permanent tooth has nowhere to erupt. Second, advanced decay in a primary tooth can affect the developing permanent tooth bud directly beneath it.

According to the World Health Organization's 2022 report, around 514 million children worldwide are affected by decay in their primary teeth. It is one of the most common chronic conditions of childhood, and largely preventable.

A first dental visit within months of the first tooth appearing is less about treatment than about building habits and spotting risk early.
