Personal Data Protection Notice

This notice is provided by Dr. Murat Papak Oral and Dental Health Clinic as data controller, pursuant to Article 10 of Turkish Personal Data Protection Law No. 6698 ("KVKK").

2. Personal Data Processed

Identity and contact data: name, phone, e-mail address.
Transaction security data: IP address, browser and device information, activity logs.
Health data: processed only at the clinic, during examination and treatment, within your patient file. No health data is collected through the website.

3. Purposes of Processing

Receiving and scheduling appointment requests,
Providing oral and dental health services and creating a patient file,
Resolving requests and complaints,
Fulfilling retention, reporting and notification obligations arising from legislation.

4. Legal Grounds

Your personal data is processed under Article 5/2 of KVKK (performance of a contract, legal obligation and legitimate interest). Health data is processed under Article 6/3 for public health protection, preventive medicine, medical diagnosis, treatment and care services by healthcare personnel bound by a duty of confidentiality.

5. Transfers

Data may be transferred to competent public authorities where legally required, to suppliers needed to deliver the service (hosting, e-mail infrastructure, laboratory) and, at your request, to healthcare institutions you are referred to. Any transfer abroad within cloud services takes place in accordance with Article 9 of KVKK.

6. Method of Collection

Data is collected through website forms, telephone, e-mail and forms completed at the clinic, by partly automated and non-automated means.

7. Data Subject Rights (Article 11)

You have the right to learn whether your data is processed, request information about it, learn the purpose of processing, know the third parties to whom it is transferred, request correction of incomplete or inaccurate data, request erasure or destruction where the conditions are met, request notification of these actions to third parties, object to results arising from automated analysis, and claim compensation for damage caused by unlawful processing.
